Rachel Liber, LCSW, CCTSW
Great to meet you! If you're a transplant patient, caregiver, or living donor and struggling, you're not alone -- and you don't have to figure this out on your own. I'm Rachel, a Licensed Clinical Social Worker and Certified Clinical Transplant Social Worker. I specialize in supporting transplant connected individuals navigating anxiety, addiction. depression, grief, medical trauma, and caregiver burnout.

I offer therapy that's warm, participatory, inquisitive, and I'm not afraid to keep it real.

I've been a social worker for over 13 years. I've worked both in direct patient care and in nonprofit management. I'd worked in hospital settings when I found my way to solid organ transplant. I spent three years as a liver transplant social worker, I provided coverage for other solid organs, and I led support groups. When my family relocated I had to step away from my role at a transplant center, but my passion for transplant remains. I've spent the last few years providing therapy for teens and adults, and now I get to blend these worlds by providing specialized mental health care for transplant patients, caregivers, and living donors.

A Bit More About Me:
I earned my undergraduate degrees in Psychology and Communications from University of Washington in Seattle.
I received my Master's of Science in Social Work from Columbia University in New York.
I'm a Licensed Clinical Social Worker in the states of California (License #120508) and Tennessee (License #9173).
I've worked with children, teens, and adults in inpatient and outpatient settings.
As a liver transplant social worker I specialized in supporting individuals struggling with addiction, and helping patients and caregivers navigate anxiety, depression, and medical trauma. I'm passionate about helping transplant connected individuals feel more understood, empowered, and equipped to handle the mental load of the transplant process.
I also have expertise in suicide prevention and intervention. I was a manager of a suicide prevention center where I oversaw the 24-hr suicide crisis line and trained staff, volunteers, community members and law enforcement.

In transplant there can be so much emphasis on the physical side of medical care -- let's work together on your mental wellbeing.
